New Video Shines a Light on How the Care Transfer Hub Supports Patients Leaving Hospital
This collaboration ensures that patients are discharged at the right time, with the right support, and in the right place.
The short film follows a patient’s journey from admission to discharge, showing how the Hub’s multi-disciplinary team of nurses, therapists, social workers, support workers and pharmacists work together to make the process smooth and reassuring.
Kirsty, Clinical Lead for the Care Transfer Hub, explains in the video:
“Our aim is to send home as many patients as we can with either a package of care or other support in the community. We work to ensure that each patient’s discharge is timely, safe, and meets their individual needs.”
The Hub’s “Home First” approach means many patients are supported to return to the comfort of their own homes, with therapy assessments, equipment, care packages and follow-up visits arranged where needed. For those who require additional support, the Hub works with community services and care providers to make sure people continue to receive the right help outside of hospital.
