Shropshire Care Closer to Home Case Managers in place

Case Managers are now in place at pilot sites. Case Manager training is now complete and the teams have taken up their positions at the designated pilot sites this week.

Pilot Sites

Seven Case Managers have started in post this week at Albrighton, Plas Ffynnon, Wem and Prees, Bridgnorth, Bishops Castle, The Meadows and Pontesbury Medical Practices. Belvidere will be going live at a slightly later date in mid-September.

To ensure that patients receive the right health and social care for their needs, the system will pinpoint individuals who are over 65 with complex health conditions and at a greater risk of hospital admission. 

The list will then be reviewed, with the help of a GP, to agree whether the needs of a particular patient can be helped via entry into the Programme. Once approved, the Case Manager then seeks patient consent and the care planning begins.

It is early days within my new role but I am excited for the future and watching how this pilot develops. I am particularly looking forward to building relationships with other members of the multidisciplinary team that I may not have previously worked with. I feel that this pilot is really looking after the local community, being proactive to our service users’ health needs and not simply being reactive.
